The Pfizer vaccine will be available for all individuals 12 years of age or older. Individuals will be able to choose which vaccine they would like to receive. Minors must be accompanied by an adult.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Participants should note that Polk State remains in Phase II of its&nbsp;<a href=\"https:\/\/www.polk.edu\/campus-safety-security\/covid-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">Return to Campus Plan<\/a>&nbsp;and all individuals on campus should wear masks and practice social distancing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In preparation for vaccination individuals are asked to:<\/p>\n\n\n\n<ol class=\"wp-block-list\" type=\"1\"><li>Download, print, and complete the&nbsp;<a href=\"http:\/\/www.polk.edu\/wp-content\/uploads\/COVID-19-vaccine-consent-form-english-5-17-21.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">COVID-19 Screening and Consent Form<\/a>&nbsp;which can be accessed&nbsp;<a href=\"http:\/\/www.polk.edu\/wp-content\/uploads\/COVID-19-vaccine-consent-form-english-5-17-21.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">HERE<\/a>.
