Main St., Lakeland: On December 22, the restaurant had 11 violations, many of which were related to the facility\u2019s process for sealing prepared foods in reduced-oxygen packaging.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The inspector observed frozen packs of raw cod, salmon cakes, cooked crab, raw salmon, raw mahi-mahi and rock grouper were not frozen before and after vacuum sealing (intermediate); a stop sale was issued on those items (high priority).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Three others were high priority: Fresh salmon was offered undercooked without proper parasite destruction; a reduced-oxygen-packaged cheddar cheese had a label date that exceeded 30 days; and cut tomatoes and cooked rice were cold held at greater than 41 degrees.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There were four other intermediate violations: no written procedures for use of time as a public health control to hold temperature-controlled food (butter); the restaurant was vacuum packaging several items without an HACCP plan approved by the Division of Hotels and Restaurants; records for required employee training didn\u2019t contain all of the required information; and required training was expired for some employees.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two others were basic, both related to the reduced-oxygen packaging violations above: One was related to labeling with times and dates; and the other was for not removing the items from their ROP package within 48 hours.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/polk-museum-of-art\/cat6306550\/2625255\/?next=\/restaurant-inspections\/polk\/\">Polk Museum of Art<\/a><\/strong>, 800 E. Palmetto St., Lakeland: On Dec. 30, the catering operation had seven violations, none high priority.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Three were intermediate: no paper towels provided at a hand wash sink; no proof of required training for any employees; and no proof that employees are informed of their responsibility to report information about their health and foodborne illnesses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Four were basic: A high temperature dish machine wasn\u2019t being used, water measured only 87 degrees; no copy of latest inspection report available; multiple opened employee drinks were in reach-in freezers; and a wooden food-contact surface wasn\u2019t properly sealed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/harrys-seafood-bar-grille\/sea6305898\/2243054\/?next=\/restaurant-inspections\/polk\/\">Harry&#8217;s Seafood Bar &amp; Grille<\/a><\/strong>, 101 N. Kentucky Ave., Lakeland: On Dec. 30, the restaurant had seven violations, including one high priority: The dish machine wasn\u2019t mixing sanitizer.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">One was intermediate: no paper towels or hand drying device at a hand wash sink.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The others were basic: Employee bags, jackets and purses were stored next to spices and single service items in dry storage; ice buildup in a walk-in freezer; no handwashing sign provided at a hand sink; fan covers were soiled with dust;&nbsp;and an ice machine was draining water onto the floor and going into the front floor grate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/urban-air-lakeland\/sea6308486\/9452402\/?next=\/restaurant-inspections\/polk\/\">Urban Air Lakeland<\/a><\/strong>, 3800 U.S. 98 N., Suite 110, Lakeland: On Dec. 22, the facility had five violations, none high priority:<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">One was intermediate: The person in charge lacked proof of food manager certification.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The others were basic: an accumulation of debris on the interior shield in an ice machine; the floor in the main kitchen was soiled; plastic water bottles were stored on the floor of a storage area; the interior of a microwave oven was soiled with food debris.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/jamaican-jerk-kitchen\/sea6308442\/7845429\/?next=\/restaurant-inspections\/polk\/\">Jamaican Jerk Kitchen<\/a><\/strong>, 210 W. 8th St., Lakeland: On Dec. 30, the restaurant had a call back for an October inspection in which six violations were found.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In the follow-up, four violations remained, none high priority: Renovations were made without a plan submitted for approval (intermediate); reach-in cooler doors had a buildup of debris (basic); food debris on the floor in the kitchen (basic); and the interior of an oven had an accumulation of food debris (basic).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An administrative complaint was recommended.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/mcdonalds-34577\/sea6308198\/6624458\/?next=\/restaurant-inspections\/polk\/\">McDonald&#8217;s #34577<\/a><\/strong>, 4220 Clubhouse Road, Lakeland: On Dec. 19, the restaurant had three violations, two of which were high priority and related to the same issue: Several items were cold held at greater than 41 degrees, including shelled eggs (56), Canadian bacon (52) and bacon (53). A stop sale was issued on those items.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The other was basic: A case of clear cups were stored on the floor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued. In a call back on Dec. 22, the restaurant met standards with zero violations.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/rg-s-lakeland-llc\/sea6307585\/9149559\/?next=\/restaurant-inspections\/polk\/\">Moe\u2019s Southwestern Grill<\/a><\/strong>\u00a0(Rg S Lakeland LLC), 1326 Town Center Drive E., Lakeland: On Dec. 30, the restaurant had three violations: Pinto beans, grilled onions and peppers were hot held at less than 135 degrees (high priority); no proof of required training provided for any employees (intermediate); and the bottom of an exterior door had a gap at the threshold (basic). A warning was issued.<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/jets-pizza\/nos6307822\/6143593\/?next=\/restaurant-inspections\/polk\/\">Jet&#8217;s Pizza<\/a><\/strong>, 3234 S. Florida Ave., Suite A, Lakeland: On Dec. 30, the restaurant had two intermediate violations: Required training was expired for all employees; and a spray bottle with a blue liquid inside was unlabeled. A warning was issued.<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/vampire-penguin-lakeland\/none\/9791474\/?next=\/restaurant-inspections\/polk\/\">Vampire Penguin Lakeland<\/a><\/strong>, 216 E. Pine St., Lakeland: On Dec. 23, the dessert shop had two violations: operating without a license from the Division of Hotels and Restaurants (high priority); and in dry storage a ceiling exposed an AC unit, duct work and exposed wiring.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">A warning was issued. In a call back on Dec. 30, the restaurant met standards with zero violations.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"https:\/\/data.theledger.com\/restaurant-inspections\/polk\/hersheys-ice-cream\/nos6308708\/8209949\/?next=\/restaurant-inspections\/polk\/\">Hershey&#8217;s Ice Cream<\/a><\/strong>, 3800 U.S. 98 N., K005, Lakeland: On Dec. 22, the restaurant had a single intermediate violation: no proof of required employee training for any employees. A warning was issued.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The terminology<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What does all that terminology in state restaurant inspections mean?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Basic violations are those considered against best practices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A warning is issued after an inspector documents violations that must be corrected by a certain date or within a specified number of days from receipt of the inspection report.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An administrative complaint is a form of legal action taken by the division. Insufficient compliance after a warning, a pattern of repeat violations or existence of serious conditions that warrant immediate action may result in the division initiating an administrative complaint against the establishment. Says the division website: \u201cCorrecting the violations is important, but penalties may still result from violations corrected after the warning time was over.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An emergency order \u2014 when a restaurant is closed by the inspector \u2014 is based on an immediate threat to the public.
